Requesting a Replacement/Duplicate Copy of Your Diploma
A replacement copy of your diploma can be ordered by logging into the Online Form using your HawkID and password or by using the Printable Duplicate Diploma Order Form. (If you graduated prior to 2003 you must use the printable form.)
Your name on the diploma will appear as it does on your UI student records unless you submit a name change request with the required documentation. All duplicate diplomas will have the notation "Duplicate diploma issued" with the issue date in the lower left corner. No duplicate diploma can be issued to former students whose University account has a financial delinquency (indebtedness).
Please allow 10 working days, following the receipt of your request, for the new diploma to be mailed to you.
USPS First Class mailings sent outside the U.S. may take 6 - 8 weeks to arrive. If you reside outside the U.S. you may want to consider having your replacement diploma delivered to you via Fed Ex. It is necessary for you to request this at the time you order your replacement diploma. Fed Ex fees can be charged to your Ubill. Cash or credit card payment cannot be accepted when ordering your replacement diploma, however, once charges have been billed, Ubill payments may be made several ways. International payments may also be made using Flywire.

Alumni who graduated in 2003 or later can order a duplicate diploma online. This requires using their HawkID and password. If your HawkID password has expired, please phone the ITS Help Desk at (319) 384-4357 to have it reset.
Printable Duplicate Diploma Order Form:
The Duplicate Diploma Order Form can be completed on your computer and printed or printed and filled out by hand.
Once printed, you must sign the form before a notary who will confirm your identity by signing the form and applying their notary seal. This protects your identity and the confidentiality of your student records. Only if your request is being mailed from outside the United States, and bears an international postmark, the notary requirement may be waived. All requests must bear the signature of the UI alum who is requesting the duplicate diploma as well as a current residing/mailing address.

Requesting Notarized Copies of University of Iowa Diplomas
You must have your original UI diploma in order to obtain notarized copies. If you do not have your original diploma you must follow the instructions above to order a replacement diploma from which notarized copies may be made.
Graduates sometimes request a replacement diploma when a notarized copy of their diploma would serve their purposes. A notarized copy is a photocopy of an existing diploma printed on security paper, notarized, and embossed with the University of Iowa's seal. This kind of documentation is often used for licensing boards, visas, etc.
There is no charge for notarized copies, however, larger orders will require additional processing time. Notarized copies require an original, full-sized photocopy or scanned image of the original UI diploma. In most cases, notarized copies will be mailed to you approximately two working days after we receive your request.
