Midterm Evening Exam Scheduling Request Form
Deadline for Spring 2010 requests is September 14, 2009

Please complete the following request form.

Filing this form allows the instructor to reserve classroom space for evening exams and to have the exam listed on ISIS to warn students that the course uses evening exams. However, instructors giving evening exams are still bound by University policy regarding exams held outside of the class time. The complete policy is printed at the bottom of this form. The key point, however, is that students may not be prevented from enrolling in or attending evening classes that conflict with exams held outside of class time.

Upon receipt of room reservation confirmation departments can place exact time and days of examination on OSRIS under course description.
