Send a notarized statement. A notarized statement confirms your identity and protects the confidentiality of your student records. Note: Only if your request is being mailed from outside the United States, and bears an international postmark, is the notary requirement waived.

Send a $45 check payable to The University of Iowa. The fee covers the cost of printing and mailing.

Please indicate the following:
- the address to which the diploma should be mailed
- your full name
- student ID number or last 4 digits of your SSN
- title of degree received
- date (month and year) the degree was awarded
Your name on the diploma will appear as it does on your UI student records unless you submit a name change request with the required documentation. (See student forms in the navigation bar to the left.)
All duplicate diplomas will have the notation "Duplicate diploma issued" with the issue date in the lower left corner.
Diplomas originally awarded prior to 1992, will bear the signatures of the current Board of Regent President, UI President and UI Dean.
Please allow up to a week following the receipt of your request, for the new diploma to be sent.
